Event Overview: 
Findhelp WV – Bridging Care: Strengthening Health Center and Community Partnerships is a  webinar designed to bring together community-based organizations (CBOs) and health centers serving Boone, Kanawha, Lincoln, Logan, Mingo, and Putnam counties. All participating organizations utilize the Findhelp platform for referrals, making this event a focused opportunity to improve how the system is used to connect individuals to care.
The webinar will center on identifying and addressing barriers that impact patient referrals between health centers and CBOs, while strengthening working relationships across organizations. By fostering open dialogue and shared problem-solving, participants will enhance communication, clarify roles and resources, and develop more efficient referral pathways. This collaborative effort aims to support timely, coordinated access to health and social services for individuals and families across the region.

Objectives and Goals:
  • Identify common barriers to effective referrals between health centers and community-based organizations using Findhelp
  • Strengthen understanding of partner roles, services, and referral processes across organizations
  • Improve communication and coordination to support seamless, patient-centered care transitions
  • Share best practices for using Findhelp to streamline referrals and close service gaps
  • Build sustainable partnerships that enhance access to health and community services across the six-county region

Target Audience: Closed group of CBOs and 3 health centers In Boone, Kanawha, Lincoln, Logan, Mingo, and Putnam Counties.
