The West Virginia Primary Care Association offers the Patient Experience Survey twice a year to participating health centers. The purpose of this survey program is three-fold:

- Meets certain HRSA reporting requirements towards PCMH recognition

By the end of the session participants will be able to:

Understand the steps to obtain the correct number of surveys
Understand useful tips and pointers to have a successful survey
Understand data gathered and how to address changes if needed
