Event Overview: This in-depth training will provide a step-by-step review of the process and data requirements for 2026 NCQA PCMH Annual reporting. Karen Taubert, NCQA PCMH Content Expert, will provide an overview of all the changes upcoming for 2026 reporting. Be sure not to miss out on this training, so you and your health center can be prepared for PCMH Reporting!
Objectives and Goals:
1. Understand - Attendees will identify key steps in the annual review process including QPASS set up, attestation and data reporting, submission, review, and the audit process.
2. Analyze - Attendees will conduct a high-level appraisal of their readiness for annual reporting identifying data sources, standard measures and areas of strength and weaknesses.
3. Apply - Attendees will formulate an approach to complete a successful annual renewal reporting for 2026.
Outcomes:
-Identify data sources to meet the 2026 annual renewal requirements.
-Determine the status of current processes with the organization that meet the core requirements.
-Plan a timeline to complete the annual renewal by the reporting date.

Target Audience: This event is intended for anyone at your CHC involved in the submission of the Patient-Centered Medical Home application, including nurses, quality staff, etc.
Cost: This training is being offered at a price of only $25 per participant!
This training is only open to CHC staff.
